Service Agreement: I understand that the full procedure fee is due regardless of whether the Perfection Visit is needed, attended, canceled, or declined.

If I am using an in-house payment plan, I understand that any remaining balance will still be due even if I choose not to complete the Perfection Visit or fail to attend a scheduled appointment.

I understand that a valid credit card is required to remain on file and may be charged in accordance with All About The Brow’s cancellation, late-cancellation, and no-show policies.
